If you have insurance, the fee is based on the pre-arranged reimbursement fee schedule established by the designated insurance company.

Cancellation Policy
If you are unable to attend a session, please make sure you cancel/reschedule at least 24 hours beforehand. Otherwise, you will be charged a fee. A fee will be charged for not showing up for scheduled appointment.
Services will be terminated after the third missed appointment and a referral will be provided upon request.
